Cover Picture: There is a high level of need for non-invasive, peripherally accessible biomarkers that characterize patients, disease pathophysiology, and progression, detect the effects of treatments, inform patient selection and stratification for clinical trials, and ultimately enable drug developers to make decisions about programs based on clinical data and biomarker data. We are in the “early days” of our understanding of the potential uses and impact of biomarkers in neural-derived extracellular vesicles (NDEs) for CNS drug development. There is evidence that some NDEs can be isolated from plasma samples, and their cargo is indicative of brain pathology, and that it correlates with clinical signs and symptoms of Alzheimer’s (for example). In this collaboration between an academic lab, diagnostic, and therapeutic companies, we further establish NDEs as a biomarker platform, developing spike-in tools needed for analytical validation and demonstrating biomarker potential.
