Figure 3. Second intraoperative repair. After returning to the OR for venous congestion and hematoma of the buccal space, the AV anastomosis was taken down. After exploration a suitable artery on the flap (facial artery stump) was identified and anastomosed to the donor facial artery - previously anastomosed to the facial vein. To avoid further venous congestion two facial veins were isolated on the flap and used to drain to the recipient facial vein and a mobilized retromandibular vein.
